ABSTRACT

BACKGROUND AND OBJECTIVES: Cone beam CT is a relatively new radiologic technique for head and neck imaging, and is widely used in dentistry to plan implant procedures. Compared to conventional CT, it has greater spatial resolution, a lower radiation dose (about 10% that of conventional CT), and lower cost. The objective of this study was to evaluate the utility of cone beam CT for endoscopic sinus surgery. SUBJECTS AND METHODS: A total of 50 patients were involved. Endoscopic sinus surgery was performed unilaterally under local anesthesia, so we used the half Lund-Mackay score to quantify preoperative CT findings. The intraoperative severity score was also determined, and the relationship between the preoperative Lund-Mackay score and intraoperative severity was assessed. The subjective level of operator satisfaction was scored on a ten-point visual analogue scale in terms of four items: overall satisfaction, disease extent of each sinus, identification of anatomic landmarks, and distinction of soft tissue characteristics. RESULTS: The preoperative half Lund-Mackay score was 5.26 and the intraoperative severity score was 5.02; these two scores were significantly correlated (p < 0.01). The level of overall operator satisfaction was relatively high (9.04/10). CONCLUSION: Cone beam CT is a useful imaging method for preoperative evaluation of endoscopic sinus surgery.

ABSTRACT

BACKGROUND AND PURPOSE: This purpose of this research is to study the prevalence of habitual snoring and the association between snoring and cephalo-anthropometric analysis in Korean population. MATERIAL AND METHOD: Four thousand and two hundred twenty four subjects (male: 2277, female: 1947), aged between 18~85 (mean age male: 45.3, female: 44.0), were recruited through home visits and interviewed by well-trained investigators. RESULTS: Three hundred fifty two male subjects (15.5%) and one hundred sixty two female subjects (8.32%) exhibited habitual snoring. Age, weight, and body mass index (BMI) were significantly higher in the snoring group compared to those who dont snore habitually. Using a stepwise variable selection method of multiple logistic regression analysis, we found independent factors as being significant for males and females of the snoring groups. For males, the factors were BMI (p<0.001, OR=1.18 CI: 1.12~1.25), mandibular angle (p<0.001, OR=0.97 CI: 0.96~0.98), and abdominal girth (p<0.001, OR=1.04, CI: 1.02~1.06. For females, they were abdominal girth (p=0.031, OR=1.03, CI: 1.00~1.05), neck girth (p=0.003, OR=1.12, CI: 1.04~1.21), age (p=0.0002, OR=1.02 CI: 1.01~10.3), and BMI (p=0.0007, OR=1.11 CI: 1.05~1.18). CONCLUSION: We found that BMI, gonion angle and abdomen girth in male and abdominal girth, neck girth, age, BMI in female were the significant factors affecting the severity of snoring in Korean population.

ABSTRACT

BACKGROUND AND OBJECTIVES: Injection of endotoxin into the middle ear of experimental animals results in otitis media with effusion and transient reversible hearing loss, which occurs in the higher concentration of endotoxin than the concentration found in human middle ear effusion. The purpose of this study was to investigate the histopathologic changes of the cochlea in guinea pigs with experimental otitis media with effusion induced by E.coli endotoxin. MATERIALS AND METHODS: Concentrations of lipopolysaccharides (LPS) from E.coli, such as 0.1 mg/ml, 1 mg/ml, and 10 mg/ml, were instillated into the middle ear cavity of 30 normal guinea pigs. Histopathologic changes of the cochlea were observed at 1 week after endotoxin instillation, using both light and electron microscope. Six normal guinea pigs were used as a control group whose middle ears were instillated with the physiologically sterile saline. RESULTS: There were deformation in the organ of Corti, partial loss of stereocilia in the hair cells, and vacuolation in stria vascularis at a concentration of 10 mg/ml lipopolysaccharides from E. coli. Mild morphologic abnormality of the cochlea was observed at the concentrations of 1 mg/ml and 0.1 mg/ml. CONCLUSION: A higher concentration of LPS in the middle ear cavity may produce severe inner ear damage, possibly resulting in the sensorineural hearing loss.
